Designers pay attention to many things including lines, textures, proportions, and how elements interact with one another. Sketching, doodling, even scribbling, can help streamline tangential thoughts about a design, can help organize the hierarchy of objects within a space, and can help a designer decide how things will feel before anyone touches a thing. Lighting affects everything, but so does line weight, shape, layering, grouping, tonality, and hue. Sketching helps get these ideas out in a way that not only helps a designer communicate but also organize their thoughts.
